Qualifications

  • Bachelor's degree in healthcare or related field is required.

  • FARS and CFARS certification must be obtained within first 2 weeks.
  • 2 years' experience in a healthcare setting is required.
  • Bilingual in English and Spanish preferred.

Job Summary:

The Licensed Clinical Social Worker assists patients and their families understand a particular illness, work through the emotions of a diagnosis, and provide counseling about the decisions that need to be made. Works in collaboration with CHC clinical staff (doctors, nurses, and allied health professionals) using case management skills to help patients and their families address and resolve the social, financial and psychological problems related to their health condition.

Primary Responsibilities and Specific Duties:

  • Initials screening and evaluation of patient and families, including but not limited to comprehensive psychosocial assessment of patients.
  • Helps patients and families understand the illness and treatment options, as well as consequences of various treatments or treatment refusal. Helps patients/families adjust to hospital admission; possible role changes; exploring emotional/social responses to illness and treatment.
  • Educates patients on the roles of health care team members; assists patients and families in communicating with one another and to members of health care team; interpreting information.
  • Educates patients on the levels of health care (i.e. acute, sub- acute, home care); entitlements; community resources; and advance directives; facilitating decision making on behalf of patients and families.
  • Assesses, plans, arranges, and implements psychosocial programs.
  • Responsible for the content of education and counseling programs for both individual patients or groups of patients and serves as liaison to community, individuals and groups as appropriate.
  • Performs short term counseling and crisis intervention as necessary.
  • Assists in the implementation of community and social programs as a professional counselor in accordance with CHC grants/contracts with federal, state, and local agencies and mandates of service requirements with those grants/contracts.
  • Coordinates with clinical, health education, community health and other team members in the provision of services to patients. Provides referrals for services needed.
  • Uses tests, measurements, and measurement scales for general and specific assessments as well as objective patient criteria in the provision of services and evaluation procedures.
  • Provides in-service presentations to team members as needed or required.
  • Diagnoses underlying mental illness; providing or making referrals for individual, family, and group Psychotherapy.
  • Promotes communication and collaboration among health care team members.
  • Coordinates patient discharge and continuity of care planning.
  • Arranges for resources/funds to finance medications, durable medical equipment, and other needed services.
  • Ensures communication and understanding about post-hospital care among patient, family and health care team members.
  • Advocates for patient and family needs in different settings: inpatient, outpatient, home, and in the community.
  • Performs other related duties as assigned.

Qualifications:

Education:

  • Bachelors degree in healthcare or other related field.
  • Masters degree in social work from a Council on Social Work Education accredited school of social work.

Certification/Licensure:

  • LCSW - License as Clinical Social Worker
  • Current BLS certification required or must be obtained within the first 30-days of employment.
  • Must obtain the Functional Assessment Rating Scale (FARS) and Children's Functional Assessment Rating Scale (CFARS) certification within the first 2 weeks of employment.

Experience:

  • 2 years previous experience in a healthcare setting is required.
  • Experience in an ambulatory health center preferred.
  • Requires someone familiar with and sensitive to Hispanic culture, family customs and child rearing practices.
  • Familiarity with multicultural groups, previous work experience in a multicultural setting and willingness to enroll language and multicultural coursework to supplement training if necessary.
  • Previous supervisory experience preferred.
  • Knowledge of EHR software or equivalent, preferred.
  • Bilingual in English and Spanish preferred.
  • Maintain confidentiality regarding privileged administrative and client information in a professional manner.
  • Intermediate knowledge of Microsoft Office, including Word, Excel and Outlook.

Other requirements:

  • Able to work flexible hours including weekends as needed.
  • May be required to travel to other CHC centers.
