Role Overview

The Patient Navigator guides patients through the healthcare system, focusing on managing and coordinating medical referrals to ensure timely access to specialty care and services. This role involves facilitating referral processes, addressing barriers to care, and educating patients about insurance, appointments, and follow-up activities.

Key Responsibilities
  • Coordinate and process patient referrals to various healthcare resources.

  • Track referral status and follow up with patients and providers to ensure completion.
  • Educate patients on referral procedures, insurance requirements, and preparation.
  • Act as a liaison, assisting with scheduling, transportation, and language needs.
  • Accurately document referral activities and patient interactions in the EMR system.
Additional Duties
  • Participate in community outreach programs about healthcare services.
  • Stay informed on healthcare policies affecting patient navigation.
  • Support projects aimed at improving care coordination.
Qualifications & Skills
  • High School Diploma; Bachelor's in Healthcare Administration, Social Work, Nursing, or related field preferred.
  • Excellent interpersonal, communication, and problem-solving skills.
  • Ability to handle sensitive information confidentially.
  • Proficiency in EHR systems and patient management software.
  • Knowledge of healthcare policies, insurance processes, and patient advocacy.
  • Certification in patient navigation, case management, or healthcare advocacy is a plus.
