Pinnacle Fertility, Inc. is hiring a Patient Navigator to guide fertility patients through their treatment journey.

This on-site, full-time role in Chicago engages with a multidisciplinary team to ensure personalized, compassionate patient care from initial contact through follow-up. You will coordinate appointments, educate patients on procedures and insurance basics, and support families as they navigate complex medical paths.

Qualifications

• 2+ years in a clinical healthcare setting providing patient care coordination.
• Strong knowledge of healthcare systems and insurance processes.
• Excellent communication, organization, and problem-solving abilities.
• Proficiency with EMR and healthcare software.
• Bilingual abilities are a plus.

Responsibilities

• Serve as primary patient contact guiding treatment plans and education.
• Coordinate appointments, tests, and referrals for care continuity.
• Explain medical care, insurance coverage, and available resources to patients.
• Collaborate with healthcare providers, clinical staff, and admin teams.
• Maintain accurate EMR documentation of interactions and plans.
• Provide emotional support to patients and families.
• Participate in team meetings and quality improvement initiatives.
• Educate patients on treatment protocols and self-care.
• Track progress, follow up on missed appointments, ensure compliance.
• Advocate for patient needs and resolve barriers to care.
• Assist with after-hours call coverage on a rotating basis.
• Support other duties and special projects as assigned.

About the Role

The Patient Navigator is an ideal opportunity for those seeking to be an integral part of a fertility patient’s journey toward becoming a parent. The Patient Navigator position at Pinnacle will allow you to engage with the patient throughout their journey while working closely with a multidisciplinary team to provide excellent patient care.

We are seeking a Patient Navigator to join our dedicated team at Pinnacle Fertility - Illinois located in Chicago, IL. This is a full-time, on-site position working from Monday-Friday from 8:30 AM - 5:00 PM schedule.

Key Responsibilities

• Serve as the primary point of contact for patients, guiding them through their treatment plan and providing education on procedures, testing, and follow-up care.
• Coordinate and schedule patient appointments, tests, and referrals to ensure a smooth continuum of care.
• Assist patients with understanding their medical care, insurance coverage, and available financial resources.
• Communicate with healthcare providers, clinical staff, and administrative teams to ensure seamless patient care coordination.
• Maintain accurate and up-to-date patient records, including documentation of interactions and care plans in the electronic medical record (EMR) system.
• Provide emotional support and resources to patients and their families, addressing any concerns or barriers to care.
• Participate in team meetings, case conferences, and quality improvement initiatives to enhance patient care delivery.
• Educate patients on treatment protocols, self-care strategies, and medication usage.
• Track patient progress, follow up on missed appointments, and ensure compliance with treatment plans.
• Advocate for patients’ needs and help resolve any issues or barriers to care.
• Assist with after-hours call coverage on a rotating schedule, ensuring patient needs are addressed appropriately.
• Other duties and project as assigned

Position Requirements

• Experience: Minimum of 2 years of experience in a clinical healthcare setting, including patient care coordination, patient navigation, or case management.
• Skills:
  • Strong knowledge of healthcare systems, insurance processes, and patient advocacy.
  • Excellent communication, organizational, and problem-solving skills.
  • Proficiency in using electronic medical records (EMR) and other healthcare software.
  • Bilingual skills are a plus.

COMPENSATION & BENEFITS

• Hourly Rate: $24.00 - $26.00 (Final offers based on experience, skills, and qualifications.)
