Job summary

CommonSpirit Health is seeking an experienced Patient Logistics RN in Rancho Cordova, CA. You will coordinate transfer center activities and act as the primary liaison for patient transfers and admissions to ensure proper placement.

You will collaborate daily with hospital administration, nursing leadership, medical staff, physicians, and transport vendors to prioritize bed assignments based on diagnoses, required level of care, and acuity.

Qualifications

• RN with 3 years of clinical hospital experience including emergency and/or critical care.
• CA Registered Nurse license required.
• Ability to coordinate transfers and prioritize patient placement.

Job Summary and Responsibilities

As our Patient Logistics RN, you will coordinate transfer center activities for our facilities, serving as the primary liaison for patient transfers and admissions to ensure appropriate placement.

Every day you will collaborate with hospital administration, nursing leadership, medical staff, physicians, and transportation vendors to facilitate bed assignments. You will participate in all activities to prioritize patient placement, basing facility designation on diagnoses, procedures, level of care, and acuity.

To be successful in this role, you will be a Registered Nurse with strong coordination and communication skills, a deep understanding of patient throughput and bed management, and the ability to collaborate effectively with diverse stakeholders to ensure appropriate and timely patient placement.

• Collect objective/subjective data for effective and appropriate transfers of patients
• Determine facility designation based on diagnoses, planned procedure and level of care requirements
• Maintain constant communication with both internal and external sources of information concerning patient transfers using proper etiquette.
• Utilize nursing knowledge and assessment to prioritize patient transfers based on urgency of patient’s condition, utilization of available beds.
• Communicate effectively with hospital staff to allow for patient flow. Responds efficiently during emergency situation for appropriate transfer of all patients
• Make judgments, decisions and modify patient transfer priorities based on evaluation. Include reporting abnormal or change in patient condition to appropriate personnel in a timely manner
