Overview

Patient Logistics RN - CA role at CommonSpirit Health. The Patient Logistics RN supervises the transfer center activities for Dignity Health Facilities and serves as the supervisor during the assigned shift to support the department and staff in carrying out the patient logistics program responsibilities. The RN collaborates with customers and staff to ensure efficient and effective communication with hospital administration, nursing leadership or their designee, medical staff, physicians and transportation vendors to ensure patients are assigned rooms expeditiously using all critical resources within the organization. Primary responsibility includes completing quality assurance reviews of work performed and communicating outcomes and any identified issues or barriers to the department manager and director.

Responsibilities
  • Compiles data for quality assurance and compliance with organizational and department policies, work aids, and algorithms
  • Holds other Patient Logistics staff accountable to established standards
  • Accurate and timely distribution of daily reporting
  • Participate in Patient Logistics growth and improvement
  • Support the program director and manager in preparation of reports, program material, research, data collection and analysis for various meetings and programs
  • Manage Transfer Center calls in accordance with established work aids and algorithms and support staff in learning and upholding these processes
  • Provide supervisory support to Patient Logistics RN and other staff as needed
  • Function and lead independently with self-initiative and discipline
  • Other duties as assigned to meet program and corporate needs
  • Collect objective/subjective data for effective and appropriate transfers of patients
  • Determine facility designation based on diagnoses, planned procedure and level of care requirements
  • Maintain constant communication with internal and external information sources concerning patient transfers
  • Utilize nursing knowledge to prioritize transfers based on urgency and bed utilization
  • Communicate effectively with hospital staff to support patient flow; respond efficiently during emergencies for appropriate transfers
  • Evaluate effectiveness of patient transfer in collaboration with the facility
  • Make judgments and adjust transfer priorities based on evaluation
  • Perform legible, timely, concise and accurate documentation according to policies and procedures
  • Proficient and competent with computerized documentation
  • Accept responsibility and accountability for own decisions and behaviors
  • Recognize and cooperate with direction from facility leadership
  • Participate in orientation of new staff
  • Serve as a resource to staff from other hospitals and vendors
  • Continue education by attending non-mandatory education offerings specific to area of practice
  • Maintain awareness of issues related to the nursing profession
  • Provide measurable written goals appropriate to level of experience during annual appraisal
  • Demonstrate effort to achieve preset goals throughout the year
  • Maintain a safe working environment by utilizing appropriate resources, protocols, procedures, and communication to appropriate personnel

Qualifications
Minimum Qualifications
  • Current California RN license
  • Proficiency with Microsoft Office applications
  • Familiarity with TeleTracking, Cerner and Midas
  • Excellent verbal and electronic communication skills, including professional telephone and email etiquette
  • RN with 3 years of clinical experience, including at least one year in an acute hospital setting
  • Ability to lift a minimum of 20 pounds
  • Ability to communicate effectively with a variety of customers including patients, providers, hospital and office staff, health plans, medical groups, internal departments, community resources, and peers
  • Ability to work independently and apply critical thinking toward decision-making, as well as in a team environment
  • Ability to multi-task and prioritize
  • Detail oriented with excellent organizational skills and ability to meet timeframes
  • Ability to learn quickly and retain complex information
  • Travel as needed
Preferred Qualifications
  • Progressive responsibilities leading up to or including a supervisory role
  • Experience in Utilization Management, Case Management or Care Coordination; Managed Care experience preferred
Pay Range

$47.41 - $70.52 /hour
