Responsibilities

This is an onsite position in our Phoenix, AZ Command Center for a transfer center Patient Logistics Coordinator.

Position Summary

The Patient Logistics Coordinator performs activities delegated by the Transfer Center RN and Patient Logistics Manager to facilitate the appropriate placement of patients within Dignity Health GSSA and specialized facilities. The incumbent uses effective interpersonal skills in all interactions involved with the position including explaining care needed as directed by the Transfer Center RN, sending physician, or receiving physician. This position assists with coordinating patient movement throughout Dignity Health facilities and specialized care facilities, specifically inpatient psychiatric. The Coordinator supports the transfer of care plan through effective communication with care providers and administration. Reports to the Patient Logistics Manager.

Core Duties
  • Works independently to facilitate patient placement as directed by the Patient Logistics Manager, RN, and physician orders.
  • Facilitates transfer of patients to appropriate facilities and units.
  • Identifies potential problems and reports to supervisors to prevent or resolve barriers to timely patient placement and transfer.
  • Communicates with referring and receiving facility staff to ensure timely placement at the correct level of care.
  • Documents activities accurately to promote effective communication with the care team, including Transfer Center, EDs, hospital units, leadership, and receiving facilities.
  • Uses effective communication skills with colleagues, customers, physicians, and care team members to facilitate patient placement.
  • Willingly provides and accepts constructive feedback.
  • Supports Dignity Health and department Mission and Vision statements.
  • Maintains a customer-oriented approach, focusing on internal and external relationships and exceeding expectations.
  • Participates in and encourages innovative approaches to duties.
  • Performs duties independently with minimal supervision.
Qualifications
Minimum Required
  • Hospital setting experience, preferably 1 year in acute care.
  • Proficiency with Google Suite applications.
  • Ability to navigate hospital software and type at least 40 words per minute accurately.
  • Excellent verbal and electronic communication skills, including professional etiquette.
  • Flexible and adaptable to change.
  • Ability to lift 20 lbs.
Preferred/Desired
  • Over 1 year experience in healthcare.
  • Familiarity with Cerner and TeleTracking systems.
Minimum Skills Required
  • Experience in healthcare, especially in bed control or transfer center roles.
  • Effective coordination with various stakeholders including providers, staff, health plans, and community resources.
  • Knowledge of care levels and medical terminology.
  • Independent work capability, critical thinking, multitasking, and prioritization skills.
  • Attention to detail, organizational skills, and ability to meet deadlines.
  • Quick learner with the ability to retain complex information.

Pay Range

$22.35 - $30.73 per hour
