Job summary

Beth Israel Lahey Health’s Home Infusion Services seeks a Patient Liaison who will be the face of the program across hospital sites. Qualifications

• High School diploma or GED required.
• Certification or experience as Medical Assistant or Pharmacy Technician preferred.
• 1–3 years healthcare experience in patient services, healthcare or pharmacy settings preferred.

Responsibilities

• Serve as primary contact for hospital discharge planners, case managers, and prescribers for the home infusion pharmacy.
• Meet patients referred for home infusion to assess eligibility for therapy.
• Explain insurance coverage, out-of-pocket costs, and payment options to patients.
• Educate patients/caregivers on safe medication administration and infusion equipment.
• Collaborate with nurses and pharmacists to ensure clinical documentation supports payer approval and safe transitions.
