Responsibilities

• Trains & mentors' staff.
• Create patient specific care plans and document progress towards goals.
• Direct the safe administration of medication in home or alternate site setting.
• Coordinate discharge activities and communicate with hospital and pharmacy teams.
• Identify patients capable of self-administration and educate patients & caregivers.
• Participate in on-call rotation for after-hours patient support.
