Hospice of the Valley is seeking a New Patient Coordinator to provide exceptional customer service while handling new patient referrals for the Geriatric Solutions program. You will verify insurance eligibility with the billing team and create patient records in the EMR, ensuring smooth first appointments and complete intake forms.

Qualifications

• High school diploma or GED required.
• Knowledge of medical terminology.
• Proficient in Electronic Medical Records (EMR) and Microsoft Office Suite.

Responsibilities

• Serves as expert resource on Geriatric Solutions services.
• Manages referral process to ensure referrals are identified and processed in a timely manner.
• Creates patient accounts in the EMR system.
• Coordinates with billing to verify, document, and communicate commercial insurance eligibility.
• Schedules patient first appointments with Medical Assistants and/or Schedulers.
• Ensures all patient intake forms are completed and filed.
• Monitors referrals via fax, email, phone, and web submissions.
• Maintains New Patient files and documentation.
• Handles confidential information in compliance with HIPAA.
