Job summary

Northwestern Medicine is seeking a Physical Therapist to deliver patient-centered care across our Rehabilitation Services. You will manage the course of physical therapy from initial contact to discharge, adhering to department guidelines and EMR documentation.

Qualifications

• Bachelor’s Degree in Physical Therapy required.
• Licensed Physical Therapist in Illinois required.
• BLS certification from American Heart Association required.

Responsibilities

• Demonstrates NM values of teamwork, patients first, integrity, and excellence.
• Maintains primary responsibility over patient plans of care through collaboration with PT/PTA.
• Documents evaluations, progress notes, and discharge summaries in EMR.
• Performs re-evaluations and adjusts treatment plans as needed.
• Provides educational materials suitable to the patient’s level and motivation.
• Works with the healthcare team to ensure quality patient care.
