Job summary

AccentCare, Inc. is seeking a Patient Family Liaison (Non-Clinical) in Miami-Dade County, on-site. Schedule is Thursday through Sunday for 10 hours per shift.

Responsibilities include coordinating with Admissions and Marketing, maintaining family information, and ensuring clear communication with physicians and hospice staff to support patient and family needs. The role emphasizes confidentiality under HIPAA, collaboration with care teams, and supporting patients and families in navigating

Qualifications

• Associates degree or higher preferred.
• Three to five years of professional sales experience preferred.
• Excellent communication and interpersonal skills.

• Communicate with providers, attending physicians, hospice staff and other agencies to ensure excellent provider relations.
• Help patients and families understand, accept, and follow medical recommendations; provide services to assist social, spiritual and health care adjustments.
• Partner with staff on in-services and education related to advanced care planning.
• Maintain confidentiality of sensitive information per HIPAA regulations.

As a Patient Family Liaison, you will:

• Collaborate with Admissions Department and Marketing staff to manage patient family information and sign-on visits.
• Communicate appropriately with providers, attending physicians, other appropriate hospice staff members and other agencies as needed to ensure excellent provider relations.
• Help the patient and family understand, accept, and follow medical recommendations. Provide services planned to assist the patient and family in achieving the optimum social, spiritual and heath care adjustments within their capacity.
• Partner with staff on in-services, and education related to advanced care planning.
• Maintain confidentiality of sensitive information per Health Insurance Portability & Accountability Act
