Job summary

AccentCare in St. Paul, MN is seeking a Full-Time Patient Family Liaison (Non-Clinical) to support patients and families receiving hospice care.

You will collaborate with Admissions and Marketing, communicate with providers and staff, and help families understand and follow medical recommendations while protecting privacy. This role offers Monday–Friday schedules with flexible options and a weekend rotation; hours are 9:00 am–6:00 pm with a 1-hour lunch.

Qualifications

• Associates degree or higher preferred.
• Three to five years of professional sales experience preferred.

Responsibilities

• Collaborate with Admissions Department and Marketing staff to manage patient family information and sign-on visits.
• Communicate appropriately with providers, attending physicians, other hospice staff members and other agencies as needed to ensure excellent provider relations.
• Help the patient and family understand, accept, and follow medical recommendations; provide services to assist the patient and family in achieving optimal social and health care adjustments.
• Partner with staff on in-services and education related to advanced care planning.
• Maintain confidentiality of sensitive information per HIPAA regulations.
