AccentCare, Inc. is seeking a Patient Family Liaison (Non-Clinical) in St. Paul, MN to join a purpose-driven hospice team.

The role is full-time, with a Monday–Friday schedule, flexible options, and a weekend rotation. The position involves coordinating with Admissions and Marketing, ensuring accurate patient family information, and providing compassionate support to patients and families. You will work to help patients understand medical recommendations, assist in social and spiritual

Qualifications

• Three to five years of professional sales experience preferred.
• Associates degree or higher preferred.

Responsibilities

• Collaborate with Admissions Department and Marketing staff to manage patient family information and sign-on visits.
• Communicate appropriately with providers, attending physicians, other hospice staff members and other agencies as needed to ensure excellent provider relations.
• Help the patient and family understand, accept, and follow medical recommendations; provide services to assist social, spiritual and health care adjustments.
• Partner with staff on in-services and education related to advanced care planning.
• Maintain confidentiality of sensitive information per HIPAA regulations.

• Partner with staff on in-services, and education related to advanced care planning.
• Maintain confidentiality of sensitive information per Health Insurance Portability & Accountability Act
