AccentCare is seeking a Clinical Liaison VIPC, Inpatient Hospice Admissions RN to support patients and families during meaningful moments. Location varies within hospital settings; this PRN role emphasizes weekend coverage with an hourly rate of $45-$50 depending on experience.

In this position you will identify patient needs, coordinate admissions, and collaborate with hospital teams to ensure timely transitions to hospice care, maintaining compassionate, ethics-driven care.

Qualifications

• One year of RN experience with knowledge of home health regulations and Medicare requirements.
• Medical sales or clinical outreach experience preferred.
• Strong communication and relationship-building skills.
• Ability to work independently in a hospital environment.
• Active RN license and valid driver’s license with insured vehicle.
• Passion for serving with dignity and empathy.

Responsibilities

• Serve as the primary clinical resource for hospice education, eligibility, and appropriateness.
• Conduct patient assessments and coordinate the admission process.

Clinical Liaison VIPC, Inpatient Hospice Admissions RN

Location: U of M / Southdale / St. John's

Position Type: PRN - as needed

Salary: $45 - $50 / hour (Based on of Experience)

Schedule: PRN as needed - primary need is on the weekends Sat/Sun 8am-5pm (some flexibility on scheduled hours)

As a Clinical Liaison VIPC, Inpatient Hospice Admissions RN, you will play a key role in identifying patient needs and facilitating seamless transitions into hospice services.

In this role, you'll need the heart of a nurse and a polished business savvy to interact with physicians and healthcare leadership. When hospital patients transfer to hospice, they may stay in the hospital, but care will transfer to the AccentCare Hospice team.
