Qualifications

• Analytical, problem-solving skills with attention to data integrity.

Responsibilities

• Receive, triage, and process incoming communications from multiple channels.
• Initiate intake of new orders by gathering patient demographics, prescriber info, and equipment details.
• Determine scope and apply appropriate workflows for intake, escalation, or follow-up.
• Respond to inquiries about Synapse Health with clarity and empathy.
• Act as a patient advocate to address member needs and barriers to care promptly.
• Deliver a high-quality patient experience while meeting performance goals.
• Enter, update, and maintain data in internal systems and documentation platforms.
• Maintain HIPAA compliance and data confidentiality.
• Support escalated requests with leadership and senior team members.
• Identify process gaps and contribute to continuous improvement.
