Job summary

Qualifications

• Ability to establish and maintain effective working relationships with physicians, managers, staff, and volunteers.
• Bilingual - strong command of English and Chinese/Spanish (preferred).

Responsibilities

• Act as a liaison between the family and care team to enhance communication.
• Advocate for the patient and communicate gaps to the care team.
• Identify LEP patients and conduct daily roundings.
• Assist with resolving care gaps and informing patients/families.
