Job summary

Maimonides Medical Center is seeking a patient outreach navigator to engage patients in self-management and close care gaps. You will linked to patient registries, schedule appointments, and maintain electronic health records to support coordinated care.

The role requires knowledge of medical terminology, experience with HEDIS, and strong Excel/Word/Access skills to prepare provider reports and ensure timely follow-up.

Qualifications

  • Knowledge of Medical Terminology and Coding.
  • Experience with HEDIS requirements.
  • Fluent in Excel, Word, and Access for reporting and data entry.

Responsibilities

  • Performs telephonic outreach based on patient registries and gaps in care reports.
  • Engages patients to participate in self-management and gap closing activities.
  • Maintains records of all outreach activity on each member/patient.
  • Schedules appointments for members/patients in the electronic appointment system with the appropriate provider.
  • Updates and maintains health maintenance templates on all patients.
  • Updates and maintains registries accurately and in a timely manner.
  • Prepares reports for individual providers of outreach activities to their assigned members.
  • Provides reminders to Providers of members who are scheduled for appointments and the services that need to be completed to meet compliance.
  • Outreach to or visits provider offices to request and/or retrieve needed documentation to update registries and electronic health records.
  • Screens members/patients for health home eligibility and ensures appropriate referral is completed.
  • Tracks hospital admissions to ensure that patients receive appropriate follow-up appointments after discharge.
  • Huddle with or communicates with patients’ care team as needed.
  • Communicates member/patient concerns to the care team as needed.
  • Works with a designated point-of-contact at each practice to ensure timely resolution of issues

Overview

Performs both telephonic and written outreach activities to engage patients on self-management and gap closing activities; and utilizes, updates and maintains patient registries. Schedules appointments for patients and updates electronic medical record as required.

Responsibilities
  • Performs telephonic outreach based on patient registries and gaps in care reports
  • Engages patients to participate in self-management and gap closing activities
  • Maintains records of all outreach activity on each member/patient
  • For members who are unresponsive to three telephonic outreach calls, Navigator will mail outreach letters and maintain records of these activities
  • Schedules appointments for members/patients in the electronic appointment system with the appropriate provider
  • Updates and maintains health maintenance templates on all patients
  • Updates and maintains registries accurately and in a timely manner.
  • Prepares reports for individual providers of outreach activities to their individual assigned members
  • Provides reminders to Providers of members who are scheduled for appointments and the services that need to be completed to meet compliance
  • Outreach to or visits provider offices to request and/or retrieve needed documentation to update registries and electronic health records

Qualifications
Education

High School Diploma.

Bachelor’s degree preferred.

Pay Range
