Job summary

Legacy Community Health in Houston, TX is seeking a Patient Educator - Float to translate health information into practical actions. You will educate patients one-on-one, in co-visits, and in group settings, supporting disease self-management and goal-setting while performing point-of-care testing as assigned.

As an integrated member of the clinic and care team, you’ll tailor education to diverse communities, document services in the electronic health record, and help patients access Legacy

Qualifications

  • High School Diploma or equivalent is required.
  • 3-5 years of relevant experience.
  • Bachelor’s Degree in a related field preferred.

Responsibilities

  • Implement health education programming in one-on-one, co-visit, and group settings to increase patient knowledge and self-efficacy.
  • Deliver education on core topics determined by clinic assignment; adapt to patient/community needs.
  • Assist patients in setting health-related goals based on assessments and readiness for behavior change.
  • Conduct point-of-care testing (e.g., blood glucose, HIV, pregnancy) and health screenings as assigned.
  • Document patient services in the EHR and required data systems.
  • Travel to different Legacy clinics as directed.

Patient Educator - Float (Job Overview)

Schedule: Monday-Friday (8AM-4:30PM or 8:30AM-5PM)

As a Patient Educator - Float, you’ll have the opportunity to build meaningful connections with patients and help transform health information into practical, achievable action. Through education, disease self-management, goal-setting, motivational interviewing, and other proven techniques, you’ll encourage patients as they work toward greater wellness, disease prevention, and improved health outcomes.


You’ll be an integrated member of the clinic and care team, bringing compassion, knowledge, and a commitment to meeting people where they are. By providing tailored education, point-of-care testing, and guidance through Legacy services, you’ll help make community healthcare more accessible, understandable, and empowering.

Key Responsibilities


  • Implement health education programming in one-on-one, co-visit, and group settings to increase patients’ knowledge and self-efficacy in preventing illness, managing health conditions, and improving overall wellness.

  • Deliver education on core topics determined by the clinic assignment, recognizing that topics may change based on patient and community needs.

  • Assist patients in setting health-related goals based on assessments of behavioral, social, and medical needs, readiness for health behavior change, and desired outcomes.

  • Conduct point-of-care testing, such as blood glucose, HIV testing, and pregnancy testing, along with health screenings and risk assessments.

  • Guide patients in navigating the system of care to help them achieve their health goals.

  • Document all patient services in the electronic health record and other required data systems.

  • Serve as a member of an interdisciplinary clinic and care team while promoting effective working relationships with internal and external stakeholders.

  • Identify, develop, and maintain presentations, handouts, equipment, models, teaching tools, and other instructional materials that effectively convey information to patients and community members.

  • Participate in the design and testing/evaluation of new health education curricula and teaching materials that support patient, clinic, and community needs.

  • Meet assigned performance metrics, including those related to grant/contract requirements, if applicable.

  • Participate in clinic, team, departmental, and agency meetings, trainings, and workshops as directed.

  • Travel to different Legacy clinics as directed.

  • Perform other duties as assigned.


Minimum Qualifications


  • High School Diploma or equivalent


Required Years of Experience


  • 3 - 5 years of relevant experience


Desirable Education and/or Experience


  • Bachelor’s Degree in a related field with a minimum of 1 year of relevant experience preferred.


Required Knowledge, Skills, and/or Abilities


  • Experience providing direct education to clients of diverse cultures and backgrounds in one-on-one and/or group settings.

  • Experience in a healthcare setting.

  • Experience with computers and computer programs.

  • Ability to provide point-of-care testing within 3 months if/as required per site assignment.

  • Ability to become phlebotomy trained within 3 months of hire if/as required per site assignment.

  • Ability to demonstrate core competencies within 3 months of hire.

  • Bilingual English/Spanish


Desirable Knowledge, Skills, and/or Abilities


  • Experience working in an electronic health record system.

  • Experience working with people living with or at-risk for HIV.

  • Experience working with sexual minorities and gender non-binary persons.

  • Experience providing education about pregnancy and/or the post-partum period.

  • Experience working with people who have chronic health conditions.
