Job summary

Giving Home Health Care is seeking a full-time Patient Coordinator to join our Case Management team. The role supports our healthcare team and patients by coordinating care, communicating clearly, and ensuring resources reach those who need them.

You will accompany patients to benefit renewal appointments, visit homes to update care, and uphold HIPAA while delivering compassionate service. Experience in care navigation and MS Office is preferred.

Qualifications

• Associate degree or higher preferred; high school diploma required.
• Experience in Care Team or Patient Navigation recommended.
• Strong communication and time-management abilities.
• Proficiency with MS Office (Outlook, Word, Excel).

We have an immediate need for a full-time Patient Coordinator to join our Case Management team.

The Patient Coordinator plays a vital role in ensuring effective communication and support between our healthcare team and our patients.

Responsibilities

• The Patient Coordinator is responsible for enhancing the overall patient experience and ensuring that patients receive the necessary support and resources.
• Accompany patients to their benefit renewal appointments to advocate for the patient and the program.
• Visit patient homes and update patient care.
• Effectively communicate and provide information to patients, caregivers, and physicians.
• Understand HIPAA and maintain confidentiality.
• Display a warm and friendly attitude.

Qualifications

• Strong work ethic and relationship building skills.
• Self-motivated with the ability to manage time appropriately.
• Strong communication skills inside of the office and outside.
• Experience with MS Office products (Outlook, Word, Excel, etc.)
• Minimum associate level education preferred. (High school diploma or equivalent is required.)
• Minimum 1-3 year of experience in a Care Team or Patient Navigation role.
• Dependable transportation to be able to travel locally between locations including patient homes, physician offices and Giving offices.
• Healthcare experience preferred.
