Giving Home Health Care, LLC seeks a full-time Patient Coordinator to join our Case Management team. Responsibilities include enhancing patient experience, advocating at benefit renewal appointments, visiting patient homes, and ensuring HIPAA compliance.

Qualifications

• Strong work ethic and relationship-building skills.

• Experience with MS Office (Outlook, Word, Excel).
• Minimum associate level education; high school diploma or equivalent required.
• 1–3 years of experience in a Care Team or Patient Navigation role.
• Dependable transportation to travel locally between locations.
• Healthcare experience preferred.

Responsibilities

• Enhance the overall patient experience and ensure patients receive necessary support and resources.
• Accompany patients to benefit renewal appointments to advocate for the patient and the program.
• Visit patient homes and update patient care.
• Effectively communicate information to patients, caregivers, and physicians.

We have an immediate need for a full-time Patient Coordinator to join our Case Management team.

Responsibilities

• The Patient Coordinator is responsible for enhancing the overall patient experience and ensuring that patients receive the necessary support and resources.
• Accompany patients to their benefit renewal appointments to advocate for the patient and the program
• Visit patient homes and update patient care.

Minimum Qualifications

• Experience with MS Office products (Outlook, Word, Excel, etc.)
• Minimum associate level education preferred. (High school diploma or equivalent is required.)
• Minimum 1-3 year of experience in a Care Team or Patient Navigation role
• Dependable transportation to be able to travel locally between locations including patient homes, physician offices and Giving offices
• Healthcare experience preferred
