Responsibilities

• Provide dedicated clinical oversight to a specific vulnerable patient population.
• Act as intermediary between service lines to ensure patient-centered care.
• Assist the provider in medical management of outpatients in the clinic.
• Perform assessments, vital signs, wound care, injections, and phlebotomy.
• Coordinate care across transitions and referrals; address patient needs.
• Communicate with care teams and obtain insurance authorizations as needed.
• Educate patients and caregivers about plans, medications, and follow-up.
• Document patient teaching and respond to requests.
