• Acts as a link between patients and care providers to facilitate clinical services and empowers patients to be an active part of the care team.
• Works with patients to overcome obstacles to disease management by utilizing all available resources to improve care outcomes.
• Serves as Patient Navigator to USA Health Physician Group specialty areas/service lines, guiding patients and families throughout their care journey.
