Job summary

Genesis Home Care is seeking a Registered Nurse Case Manager (RNCM) to lead patient assessments and plan of care. You will coordinate multidisciplinary teams, perform requirements like OASIS assessments, and provide skilled nursing care at patients’ homes under physician orders.

The RNCM will document all care, communicate changes to the physician, and ensure compliance with state and federal guidelines. A valid RN license and current driver’s license are required.

Qualifications

• Minimum one year RN experience in an acute care setting.
• Home care experience is desired.
• Ability to communicate effectively with patients, families, physicians and coworkers.
• Compassionate and caring demeanor.
• Knowledge of nursing best practices.
• Valid driver's license and auto insurance in your name.

Responsibilities

• Oversee overall patient care from start to finish of episode.
• Update doctor orders in patient charts.
• Notify physician of significant changes in patient condition.
• Coordinate multidisciplinary care.
• Provide skilled nursing care at the patient’s home as ordered by the physician.
• Perform OASIS assessments to develop a care plan and adjust as needed.
• Document all clinical care and ensure compliance with Medicare guidelines.
• Drive to patient locations for scheduled visits.
