Job summary

Lubbock Family Medicine seeks a Social Worker to identify and address the medically related social and emotional needs of patients/residents. You will develop social histories and care plans, coordinate with physicians, and support post-acute care planning during the patient stay.

You will participate in interdisciplinary conferences and design social service programs, while ensuring documentation compliance and patient safety. English proficiency and licensure eligibility are required.

Qualifications

• Requires a bachelor’s degree in social work or related field.
• Licensed in the state or eligible for licensure.
• Strong communication, documentation, assessment and social work methods.
• At least one year of experience in long-term care or healthcare settings.
• Ability to read, write, and speak English.

Responsibilities

• Develops social histories and care plans addressing medically related social/emotional needs.
• Participates in interdisciplinary care plan conferences and team discussions.
• Designs social service programs meeting patient needs and adheres to regulations.
• Coordinates discharge planning, referrals, and education for staff and families.
• Maintains required documentation and ensures patient safety.
