Heritage Skilled Care is seeking an Licensed Social Worker (LSW) to provide professional social work services for residents in a long‑term care setting. The incumbent will support residents’ emotional and psychosocial needs while ensuring CMS and state compliance.
The role includes conducting comprehensive assessments, care planning with the interdisciplinary team, and documenting progress in accordance with regulations.
The Licensed Social Worker (LSW) provides professional social work services to residents in a long‑term care facility. The LSW supports residents’ emotional, psychosocial, and environmental needs; ensures compliance with federal and state regulations; and promotes residents’ rights and quality of life.
Psychosocial Assessment & Care Planning
Complete comprehensive psychosocial assessments upon admission and quarterly/as needed thereafter.
Develop, implement, and update individualized care plans in coordination with the interdisciplinary team.
Document assessments, interventions, and progress notes in accordance with regulatory standards.
Resident & Family Support
Provide counseling and emotional support to residents and families.
Assist residents with adjustment to long‑term care placement, illness, disability, or end‑of‑life issues.
Facilitate family meetings and care conferences.
Provide crisis intervention when needed.
Discharge Planning & Resource Coordination
Assist with discharge planning and transitions to home, hospital, or other care settings.
Coordinate referrals to community agencies and support services.
Help residents and families access financial, legal, and healthcare resources.
Advocacy & Compliance
Advocate for residents’ rights and dignity.
Ensure compliance with CMS, state regulations, and facility policies.
Assist with advanced directives and support ethical decision‑making.
Participate in survey readiness and quality improvement initiatives.
Education & Licensure
Bachelor’s degree in Social Work (BSW) required.
Current state licensure as a Licensed Social Worker (LSW) in good standing.
Experience
1–2 years of experience in healthcare or long‑term care preferred.
Knowledge & Skills
Knowledge of long‑term care regulations and resident rights.
Strong documentation and assessment skills.
Ability to manage sensitive and confidential information.
