Job summary

Jefferson Health in Abington, PA seeks a Specialized Diploma RN to join our patient-centered care team. You will develop care plans with patients and families, coordinate with inter-professional staff, and ensure culturally respectful, high-quality nursing care across shifts.

You will participate in education, discharge planning, and continuous quality improvement, using health information systems to document care and monitor outcomes.

Qualifications

• Specialized Diploma graduate of an accredited school of nursing is required.
• Knowledge of patient- and family-centered practice and care planning.
• Understanding of diverse populations and cultural sensitivity.
• Effective communication with patients, families and staff.
• Ability to assess patients and develop/execute care plans.
• Knowledge of discharge planning and post-acute needs.
• Experience with health information documentation and nursing records.
