• Associate degree in nursing is required.
• Massachusetts RN license is required.
• Experience in diverse populations is valued.
• Union environment experience is a plus.

Responsibilities

• Collaborates with care team to improve patient outcomes through a patient-centered care plan.
• Provides nursing care based on the nursing process and evidence-based practice.
• Performs triage and assessments via in-person, telehealth, or virtual encounters.
• Coordinates preventive services and patient education.
• Documents patient information accurately in the EHR and communicates patient status with the team.
• Promotes culturally competent care and professional teamwork.
