Job summary

Advocate Health is seeking a Social Services Assistant to coordinate patient transfers, assist with programs, and support social workers in a hospital setting. You will help with paperwork, communication with families, and ensuring confidentiality and accurate documentation.

Responsibilities

• Initiates and completes routine nursing home and facility-to-facility transfers, coordinating paperwork per policy.
• Arranges transportation for patients with Social Workers and care teams.
• Assists with Transit Plus applications and provides information on Advance Directives.
• Demonstrates sound judgment and involves Social Workers when needed.
• Supports other case-related department services as directed.
• Secures physician signatures or courier services to facilitate transfers.
• Maintains patient confidentiality per policies and procedures.
• Documents patient activities in the medical record per guidelines.
• Monitors caseload in MedSWis and assists with clerical tasks.
