Qualifications

• Ability to learn quickly and work well with a care team.

Responsibilities

• Providing patient care based on age and care criteria.
• Ensuring safe and effective patient outcomes through judgment and actions.
• Ongoing communication with patients and the care team about status and care.
• Proactive initiative to improve performance and opportunities.
• Collaborating in care plans from admission to discharge for safe transitions.
• Participating in Unit/Department PI activities.
