Responsibilities

• Provide basic patient care, including comfort, vital signs, personal care, and mobility support.
• Serve as liaison between patient and nurse to report changes.
• Deliver high-level customer service to patients, families, visitors, and staff.
• Document patient care accurately and promptly in the EHR.
• Participate in hand-offs and report department needs during shift changes.
• Assist with transfers using safe techniques and promote patient independence.
• Encourage patient participation in activities aligned with interests.
