Job summary

HonorHealth in the Phoenix area is seeking a PRN Day Shift caregiver to float among East Valley locations, including Tempe Medical Center, Four Peaks Medical Center, and Florence Medical Center. The role supports patient care under a licensed caregiver and fosters growth across the life span.

Required: High School Diploma or GED, BLS, and 6 months healthcare experience; 6 months acute care preferred. Weekend/holiday rotations with shift differentials are included; self-scheduling is available

Qualifications

• High School Diploma or GED required.
• 6 months of healthcare experience in the last year required.
• 6 months acute care experience preferred.
• BLS certification required; CNA/EMT certifications preferred.

Responsibilities

• Provide direct patient care under the supervision of a licensed caregiver.
• Observe, document, and report patient changes and maintain medical records.
• Address questions or concerns from patients, families, and health care team members.
• Perform other duties as assigned.

This position is a PRN Day Shift working 12 hour shifts from 6:00AM.–6:30PM and will be floating to our East Valley Locations:

• TEMPE MEDICAL CENTER: 1500 S Mill Avenue, Tempe AZ, 85281
• FOUR PEAKS MEDICAL CENTER: 1301 S Crismon Road, Mesa, AZ 85209
• FLORENCE MEDICAL CENTER: 4545 N Hunt Highway, Florence, AZ 85132

We offer self-scheduling for shift and weekend coverage. Scheduled workdays may vary, as there are no set or fixed days. Weekend and Holiday Rotations are required in this role. Weekend shift differentials are offered, along with evening and night shift differentials where applicable.

Responsibilities

JOB SUMMARY

Provides patient related care tasks based on the principles of growth and development across the life span of patients served in his/her independent unit. Assists, observes, and reports patient changes to licensed caregiver and maintains accurate medical record documentation relative to the care delivered.

Essential Functions

• Provides and collaborates direct patient care services under the direct supervisor of a licensed caregiver. Responsibilities may include, but are not limited to personal care, activities of daily living, vital signs, intake and output, ambulation, interventions, and implementation of selected portions of the patient care plan as directed under supervisor of nurse.
• Assists, observes and reports changes in patient condition to licensed caregiver and maintains accurate medical record documentation in relation to the care delivered.
• Addresses and/or directs questions or concerns from patients, families, caregivers, or health care team members.
• Performs other duties as assigned.

Education

• High School Diploma or GED Required

Experience

• Other Completion of Block 1 in Nursing OR Medical Assistant (MA) OR Nursing Assistant (NA) OR Emergency Medical Technician (EMT) OR Behavioral Health Technician (BHT) OR 6 months of experience in the healthcare setting within the last year Required
• 6 months in acute care experience. Preferred

License and Certifications

• Basic Life Support (BLS) - Certification Required
• Certified Nursing Assistant (CNA) - Certification Preferred
• Emergency Medical Technician - Basic (EMT) - License Emergency Medical Tech Preferred
