Responsibilities

• Record vital signs and patient data and report to RN/LPN.
• Assist with meals, bathing, grooming, linen changes.
• Position patients and assist mobility with devices and walkers.
• Collect urine/stool samples and perform finger-stick glucose testing.
• Maintain clear communication with the care team and adhere to PPE.
• Participate in performance improvement and risk management activities.
• May provide continuous patient observation when required.
