Job summary

Hackensack Meridian Health is seeking a Patient Care Technician to deliver compassionate patient care and support across care teams. You will assist with daily activities, monitor patient status, and document care with accuracy while ensuring safety and quality outcomes.

Ideal candidates have a high school diploma or GED and have completed a CNA/MA/PCT program or nursing fundamentals, or are in the HMH PCT Apprenticeship with potential transfer to the PCT role after completion.

Qualifications

• High School diploma or GED required.
• Completion of CNA/MA/PCT program or nursing fundamentals degree or HMH PCT Apprenticeship; apprenticeship path if selected.
• EMT, CMA, Home Health Aide background acceptable; demonstrates commitment to patient care.

Responsibilities

• Provide patient care based on age-appropriate criteria and safety.
• Ensure safe outcomes through clinical judgment and action.
• Communicate with patients and care team regarding patient status.
• Take initiative to improve job performance and opportunities.
• Collaborate in care plans from admission to discharge for continuity of care.
• Participate in Unit/Department PI activities.
