The primary work address is at Heights of North Houston (303 Hollow Tree Ln, Houston, TX 77090). DaVita SNF Dialysis dens have 6 chairs on-site at the Skilled Nursing Facility. The CCHT must be flexible to support other DaVita SNF Dialysis dens in the Houston metro as needed.
MUST BE CCHT CERTIFIED FOR THIS ROLE
Approximately 10% of all dialysis patients reside in a Skilled Nursing Facility and are transported to dialysis three times a week, which adds a significant burden on patients and healthcare costs. DSD Services teammates will have access to all capabilities and resources DaVita offers. Teammates must be comfortable working autonomously and collaborating across teams.
You will be responsible for the complete ongoing care of each patient to ensure their dialysis treatment stays on track. PCTs will work with dialysis RNs to provide care. The CCHT may support other SNF dialysis dens in the region as needed.
Spend most of your day in direct, one-on-one patient care to provide safe, comfortable, and hygienic dialysis treatments.
DSD Dialysis care is complex but fulfilling. Meeting or exceeding clinical outcomes is the goal. Multitasking is essential to meet the needs of patients, the service, and SNF partners.
Provide comprehensive patient care independently. Monitor patients before, during, and after dialysis, recording stats, observations, and machine setup. The role requires constant vigilance and applying core skills to impact patients' lives positively.
