Job summary

Ascension in Chilton, WI seeks a Nurse Aide to join the Med Surg Swing team on the full-time night shift. You will provide compassionate, hands-on care and support daily living activities, ensuring patient comfort and dignity throughout hospitalization.

You will collaborate with the nursing team, observe and report changes in patients, assist with safe admissions and discharges, and help maintain accurate records and a clean, safe environment.

Qualifications

• High School diploma equivalency.
• 1 year of applicable cumulative job-specific experience.
• Required professional licensure/certification can be used in lieu of education or experience, if applicable.

Responsibilities

• Deliver compassionate, hands-on care to support patients with daily living activities, ensuring their comfort, dignity, and personal well-being throughout their stay.
• Serve as a vital link to the nursing team by observing and reporting changes in physical, mental, or emotional conditions to protect patient safety and enable quick clinical interventions.
• Ensure smooth patient transitions by assisting with safe entry, discharge, and movement across the facility to maintain a seamless care journey.
• Maintain accurate, real-time records of patient care activities to support the broader medical team in delivering seamless, high-quality care.
• Create a safe, healing environment by maintaining clean, well-organized patient rooms and ensuring essential medical equipment is clean, prepared, and ready for use.

What Minimum Requirements You’ll Need

Licensure / Certification / Registration:

• BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
• Nurse Aide credentialed from the Wisconsin Nurse Aide Registry obtained prior to hire date or job transfer date required. Certified Nursing Assistant credentialed from the state of Wisconsin, and in Nurse Aide Registry with eligibility to work in a hospital setting obtained prior to hire date or job transfer date. Additional certs may be required by specific job locations.

Education

• High School diploma equivalency OR 1 year of applicable cumulative job specific experience required.
• Note: Required professional licensure/certification can be used in lieu of education or experience, if applicable.
