Job summary

VIA Health Partners in Lincolnton, NC, is hiring a Certified Medical Assistant (CMA) for full-time hospice and palliative care support. The role includes patient/family contact, scheduling visits, and maintaining medical records within a community-based, not-for-profit setting.

The ideal candidate has CMA credentials, a High School Diploma, and at least 1 year of experience, preferably in homecare or geriatrics.

Qualifications

• High School Diploma preferred, plus training/certification in related field required.
• Certified Medical Assistant (CMA) is required and must be in good standing with AAMA.
• Minimum 1 year’s experience required. Experience in homecare, working with terminally ill and/or geriatrics preferred.

Responsibilities

• Initiates initial contacts with patients and families to introduce palliative and/or primary care team and to answer questions about services.
• Assists with scheduling patient appointments and clinician visits, ensuring geographic efficiency and capacity planning.
• Maintains medical records, ensures timely and compliant documentation per VIA Health Partners guidelines.
• Attends interdisciplinary group conferences and maintains professional relationships with team members.

Essential Functions

Delivery of Patient Care

• Initiates initial contacts with patients and families to introduce palliative and/or primary care team and to answer questions about services.
• Obtains verbal consent to schedule initial Nurse Practitioners (NP) visit.
• Assists with back-office tasks in eCW including scheduling patient appointments within the appropriate geographical distance, appointment reminders, and any tasks deemed by NP.
• Prepares weekly NP schedules, such as Face to Face or virtual visits. Reviews clinician schedules daily or more frequently as needed to determine capacity opportunities and ensure productivity standards are met.
• Ensure clinicians are assigned and scheduled in the most efficient geography, maximizing clinician efficiency, utilizing the lowest possible discipline, matching skill with required care, and optimizing clinician capacity.
• Ensure missed visits are scheduled by the end of the current week or referred back to leadership for resolution.
• Works collaboratively with field clinicians to ensure all patient visits are scheduled and completed as ordered within the plan of care.
• Manage calls from patients and field staff related to scheduling issues. Refers clinical and performance-related issues to the Manager of Practice Operations.
• Monitors hospitalized patients, communicates, and ensures the team follows up as necessary.
• Completes requested schedules ensuring all orders and services (i.e. lab draws) are included as required.
• Supports branch operations and works collaboratively to ensure timely communication with patients, clinicians, referral sources, and other office staff.
• Maintains medical records, obtains supplemental record and processes document attachment.
• Runs all applicable reports and responds to workflow taking appropriate actions.
• Assists internal and external transfer of patients between service lines as appropriate.
• Maintains the upmost patient experience and connection with their patient panels and NPs they support.

Administer virtual visits in the field as appropriate.

Quick response times to patients and care team; readily available during business hours.

Interdisciplinary Group

• Attends and participates in the interdisciplinary group conferences [IDG] and special care conferences.
• Assumes responsibility for establishing and maintaining effective working relationships with team members.
• Identifies specific patient/caregiver needs and makes referrals to appropriate team members.

Documentation

Ensures accurate, complete, succinct, and timely clinical documentation in accordance with VIA Health Partners guidelines and the requirements of licensure and certification standards.

Professional and Personal Accountability

• Demonstrates competency in knowledge and skills within scope of practice.
• Ensures that VIA Health Partners professional reputation is maintained and projected.
• Assumes responsibility for professional development and staying abreast of current trends in the healthcare field. Incorporates new information and methods into practice.
• Maintains current professional licensure/certification, CPR certification and CEU requirements.
• Maintains productive and professional relationships with external agencies and healthcare providers.
• Exhibits time management skills. Able to plan workday while remaining able to respond appropriately to unforeseen obstacles.

May perform other duties as required.
