Job summary

Qualifications

• Current, unencumbered license to practice as a Licensed Practical Nurse/Licensed Vocational Nurse or Registered Nurse in the state of employment.
• High School diploma or GED equivalent.
• One or more years of administrative experience in a health care environment.

Responsibilities

• Schedules home visits for the field staff and coordinates patient care under the Clinical Manager.
• Prepares weekly clinician schedules and reviews capacity to maximize utilization.
• Ensures clinicians are assigned efficiently by geography and skill requirements.
• Schedules and resolves missed visits in a timely manner and communicates with patients and staff.
• Manages calls related to scheduling issues and supports timely communication with patients and clinicians.
• Monitors hospitalized patients and ensures necessary follow-up by the team.
• Handles transfer of patients between care centers or Hospice services as needed.
• May provide direct patient care in emergencies if licensed and qualified.
