Silver Cross Hospital in Mokena, IL is seeking a Patient Navigator to improve service delivery and patient flow. The role includes coordinating clinical staff and performing lab tests and diagnostics, while ensuring timely communication between patients, families, and providers.

Requires graduation from an accredited Medical Assistant program with at least 5 years of primary care MA experience, current CPR/BCLS, and strong communication and computer skills.

Qualifications

• Graduation from an accredited Medical Assistant program; 5 years primary care MA experience preferred.
• CPR and BCLS certifications must be current.
• Phlebotomy license or experience preferred.
• Strong communication, teamwork, adaptability and computer skills.

Responsabilités

• Phone triage: answer calls and requests from patients and staff promptly.
• Follow up on high-risk patients per provider instructions.
• Oversee messages and refills for all providers.
• Engage and motivate providers for timely responses per policy.
• Transmit physician orders to patients and pharmacies.
• Respond to messages and refills per protocol.
• Perform diagnostics: EKG, spirometry, audiometry and in-house labs.
• Handle clerical tasks: scheduling, billing issues, and chart processing.
• Manage patient flow and throughput across care locations.
