Job summary

Grace Health in southeastern Kentucky seeks a Patient Navigator to act as a liaison among patients, families, and healthcare professionals, supporting pharmacy operations under pharmacist supervision and following approved guidelines.

You will process refills, assist with prior authorizations and DME forms, document all interactions in the EMR, and help coordinate transportation, referrals, and care with providers and outside partners.

Qualifications

• Required: High school diploma or equivalent.
• Preferred: Certified Pharmacy Technician, Nursing license, or Certified Medical Assistant.
• General computer skills.
• Customer service experience.

Responsibilities

• Respond professionally to a high volume of inbound calls and inbox tasks.
• Collaborate with Pharmacists on refill reviews and follow Grace Health protocols.
• Schedule appointments following protocol or providers orders.
• Document all communication with patients and family in the EMR.
• Inform patients or designated family member of lab/diagnostic results.
• Accurately establish and maintain patient medication profiles for review by the pharmacist.
• Provide superior customer service to our patients and staff by following Grace Health protocols.
• Maintains HIPAA requirements.
• Proficient in current technology to enter/update patient demographics and process refills.
• Practice active listening to ensure customer needs are met before completing the call.
• Escalate to Pharmacist/Lead any issues impacting the patient.
• Collaborates with diagnostics, referrals, scheduling, community outreach, Grace Health pharmacies, and outside pharmacies.
• Facilitate care with transportation services, home health agencies, and physician offices.
• Assist provider with completing DME, Guardianship, FLMA, and other forms.
• Complete preauthorization and patient assistant program requests.
• Document verbal orders from providers.
• Maintain current license/certification(s), if applicable.
• Other duties as assigned.
