Job summary

Oak Street Health is seeking a Medical Social Worker to join our teams focused on psychosocial support for high‑risk patients. You will assess needs, coordinate care, and advocate within a multidisciplinary group to improve health outcomes.

You will work with diverse communities, providing education on care options and resources while navigating insurance benefits. A CSWE‑accredited MSW and state licensure are required, with US work authorization and EMR proficiency.

Qualifications

• MSW degree from a CSWE-accredited program.
• Minimally licensed in the state where OSH Center is located.
• Fluency in Spanish, Polish, Russian, or other languages spoken by the communities served.
• Proficient computer skills including Microsoft and EMR systems.
• US work authorization.

Responsibilities

• Provide psychosocial assessments, advocacy, and care coordination for high‑risk patients and their caregivers.
• Collaborate with interdisciplinary care teams to identify goals and drive care plans.
• Refer to community resources to meet patients’ psychosocial needs.
• Navigate insurance benefits with PRM to optimize plan benefits.
• Deliver both longitudinal and episodic care coordination to empower patient self-management.
• Educate patients on advance care planning and goals of care at all stages.
• Provide psychoeducation to patients and families on life stage and disease‑specific issues.
• Meet with care teams to brainstorm solutions for complex psychosocial dynamics.
• Document interventions concisely to keep the care team informed.
• Mentor Community Health Workers and provide clinical guidance on complex needs.
• Respond to crises and prioritize tasks in urgent situations.
• Perform any other duties as assigned.
