Qualifications

• Fluency in Spanish, Polish, Russian, or other languages spoken by the communities served.

Responsibilities

• Provide assessments, advocacy, and care coordination to high‑risk patients and caregivers.
• Coordinate with interdisciplinary care teams to improve health status and reduce hospitalizations.
• Educate patients on options including advance care planning and end‑of‑life care.
• Complete timely documentation to ensure visibility across care teams.
• Collaborate with Community Health Workers to address complex patient needs.
