Responsibilities

• Screens patient census to determine high-risk categorization.
• Manages high-risk patient census and coordinates with NCM or other programs.
• Assesses basic needs and refers to NCM when appropriate.
• Works with the NCM to develop and update patient care plans and attend huddles.
• Performs telephonic and remote outreach and documents in the EMR.
• Provides resources and disease management education to patients.
