Job summary

Qualifications

• 2+ years of verifiable experience in healthcare or administrative support.
• Valid Driver’s License, CPR, First Aid, and AZDPS Level I Fingerprint Clearance Card.
• Proficiency with Microsoft Office, CRMs, and EMRs.
• Customer service oriented with strong interpersonal communication skills.
• Excellent organizational, task, and time-management skills.

Responsibilities

• Coordinate patient admissions, completing intake procedures and documentation.
• Coordinate all initial medical and clinical assessments.
• Assist the Utilization Review team with prior authorization and concurrent reviews.
• Collaborate with the multidisciplinary team to develop and implement treatment plans; reassess as needed.
• Develop comprehensive discharge and aftercare plans; coordinate referrals for smooth transitions.
• Coordinate with families and outside agencies to support positive patient outcomes and ensure policy compliance.
