Responsibilities

• Handle intake of new referrals and staff schedules.
• Coordinate referrals and liaise with sources.
• Manage clinical field staff schedules and ensure timely visits.
• Enter patient information into HomeCare HomeBase per Medicare.
• Collaborate with sales and clinical management on referrals.
• Provide patient information to charts and follow up as needed.
• Follow hospital holds and update statuses.
• Perform other duties as assigned.
