Qualifications

• CPR/BLS American Heart Association (BLS) certifications required or within grace period listed by employer.
• Experience in a healthcare setting is preferred.
• Proficiency with EPIC EMR and MS Office applications (Excel/Word/PowerPoint).

Responsibilities

• Coordinate patient care plans, including scheduling appointments and obtaining referrals.
• Assist patients with insurance authorizations and connecting to community resources.
• Provide education on medical conditions, treatments and peri-operative care as directed by clinical staff.
• Communicate important details to the care team and document efforts in the medical record.
