Job summary

Giving Home Health Care, LLC is seeking a Patient Coordinator in Albuquerque, NM to act as a liaison among patients, caregivers, and clinical teams. You will conduct home visits, coordinate care plans, and ensure timely documentation and compliance across EMR systems.

The role requires 3–4 days of local travel weekly, up to 1,500 miles monthly, with competitive mileage reimbursement. A flexible schedule may be needed outside normal hours, with no weekends required.

Qualifications

• 2–5 years in customer service or healthcare case management.
• Valid New Mexico driver’s license and dependable vehicle.
• Able to travel 3–4 days per week (up to 1,500 miles/month).
• Proficient in MS Office; comfortable with tablets/laptops.
• HIPAA knowledge and ability to maintain confidentiality.

Responsibilities

• Serve as the primary liaison between patients, caregivers, clinical staff, and internal departments, ensuring clear and timely communication.
• Conduct home visits to build patient relationships and initiate homecare services.
• Attend and advocate for patients at physician appointments and coordinate DME.
• Manage a caseload of 50+ patients and oversee medical documentation and renewals.
• Ensure patients have proper DME and provide training to patients and staff.
• Coordinate weekly, biweekly, and monthly check-ins with nurses and aides.
• Collaborate with a care team under the Care Team Director.
• Update documentation in Salesforce and Igea/Axxess EMR within 24 hours.
