Job summary

Aurora Behavioral Health in Glendale, AZ seeks a compassionate Social Worker (Patient Care Manager) to join our multidisciplinary team. You will perform psychosocial assessments, create and update treatment plans, and deliver individual and group therapy, while coordinating discharge planning.

Collaborate with psychiatrists, nurses, and other clinicians to support patient care, document progress in the EMR, uphold confidentiality, and advocate for patients and families.

Qualifications

• Master's degree in social work, Counseling, Psychology, or another behavioral health-related field.
• Current CPR certification (or obtained within 30 days of hire)
• Current TB clearance
• Arizona Fingerprint Clearance Card
• Must be at least 21 years of age

Responsibilities

• Complete comprehensive psychosocial assessments.
• Develop, implement, and update individualized treatment plans.
• Facilitate individual, family, and group therapy sessions.
• Coordinate discharge planning and connect patients with community resources.
• Collaborate with psychiatrists, nurses, behavioral health technicians, and other members of the interdisciplinary treatment team.
• Document assessments, treatment plans, and patient progress in the electronic medical record.
• Participate in utilization review and ensure timely documentation.
• Advocate for patients while maintaining the highest standards of confidentiality and ethical practice.
• Assist patients and families in achieving treatment goals and successful transitions of care.
