We are hiring for a Patient Care Manager in Hospice.

This is a full-time position—approximately 70% in-office and 30% in the field. A minimum of 4+ years practicing as an RN is preferred.

Responsibilities

• Assist the Executive Administrator/Director of Clinical Operations with patient care activities to ensure safe, efficient, and compliant care according to nursing, regulatory, medical, and payer standards.
• Ensure compliance with hospice policies, procedures, and all relevant local, state, and federal regulations.
• Promote and oversee quality of care and safety for patients and staff.
• Engage in thorough problem-solving and complaint investigations.
• Provide staff in-services to promote patient safety, regulatory compliance, and address educational needs.

Education and Experience

License Requirements:

• Current RN licensure in the state of practice and at least one year of clinical experience.
• Current CPR certification.
• Valid driver’s license, insurance, dependable vehicle or access to public transportation.

Additional State-Specific Requirements:

• LA: Minimum two years of full-time RN experience; alternatively, two years of full-time hospice care experience as a Licensed Practical Nurse can substitute for RN experience.
