Summary

We are hiring for a Patient Care Manager in Hospice.

Responsibilities

• Lead, inspire, and motivate staff to provide exceptional care.
• Ensure staff are oriented and supported for success.
• Act as liaison between staff, patients, and the medical community.
• Assist with patient care activities to ensure safe, efficient, and compliant care.
• Promote quality and safety standards within the team.
• Investigate and resolve complaints thoroughly.
• Provide staff in-services on health, safety, regulatory issues, and policies.

Education and Experience

License Requirements

• Current RN licensure in the practice state and at least one year of clinical experience.
• Current CPR certification.
• Valid driver’s license, vehicle insurance, and access to transportation.

Additional State Specific Requirements

• LA: Minimum two years full-time RN experience or two years full-time hospice clinical experience as a licensed practical nurse.

Position Details

This is a full-time position, approximately 70% in-office and 30% in the field. 4+ years of RN practice preferred.
