Responsibilities

The Home Health Patient Care Manager oversees clinical services, supervises nursing care provided in patients' homes as prescribed by physicians, and ensures compliance with laws, regulations, and policies.

• Provide clinical services within your scope of practice, aligned with state laws and the patient's care plan
• Manage referrals, clinician assignments, patient orders, and start-of-care visits
• Coordinate patient benefits, medical necessity, and insurance approvals
• Assess patient needs continuously and tailor care to meet their requirements and physician directives
• Review assessments and care plans daily, offering recommendations to clinicians as needed

Educational and Experience Requirements

• Current RN licensure in the practice state
• CPR certification
• Valid Driver's License, vehicle insurance, and access to reliable transportation
